MDVIP is a membership‑based model of care that allows primary care physicians to limit their practice size to 40% fewer patients than a traditional practice - so they can spend time on what matters most: being present with their patients, having deeper conversations, and focusing on preventive, personalized care. 


Members pay an annual fee that covers an advanced Annual Wellness Exam with comprehensive screenings, diagnostics and wellness planning that go beyond what insurance typically covers.


Yes, we are contracted with most major insurance carriers. Your MDVIP Membership fee covers advanced diagnostics and screenings not typically covered by insurance. The practice continues to bill your insurance for routine medical care, sick visits and other services. 


** If you are insured by CommunityCare insurance, you will need to call the Customer Service number listed on your insurance card and request your Primary Care Provider be switched to your physician in our office before your first office visit. 


While patients who are covered by Medicaid are welcome to join the practice, our physicians are not contracted with Medicaid - which means they are unable to write referrals, and any medications, prescriptions, or procedures ordered by our physicians will be an out-of-pocket expense. Any additional lab work outside of the Annual Wellness Program covered by your MDVIP Membership will also be an out-of-pocket expense. There is a $50 cash fee per regular office visit. 


** Our recommendation for Medicaid-covered Members is to maintain a concurrent physician relationship with our practice as well as your Medicaid-assigned physician for routine medications, office visits, and referrals. 


The Annual Membership Fee covers the MDVIP Annual Wellness Program (AWP) and preventive services not typically covered by insurance. 


It covers the AWP Pt. 1 (comprehensive labs, diagnostics, and screenings) and Pt. 2 (extended one-on-one office visit with your physician to discuss the results and formulate a personalized Wellness Plan for the year ahead.)  


Your Membership also ensures longer appointments, same‑day or next‑day scheduling for acute needs and direct after‑hours physician availability by phone, among other benefits. 


 After receiving your Annual Wellness Program services, your membership fee may be reimbursable through Health Savings Accounts (HSA), Flexible Spending Accounts (FSA) or similar programs. 


 As a member of our practice, you can usually schedule same‑day or next‑day appointments for acute needs and enjoy minimal waiting times.


Yes. Your physicians have the availability to coordinate care with specialists on your behalf, ensuring continuity of care and ongoing communication with any specialty providers. 


For more complex needs that require services beyond local/regional specialist care, MDVIP’s Medical Centers of Excellence program assists your physician in connecting you with nationally recognized medical institutions and specialists when needed.

 Yes, in most cases you will need to call our office to schedule an appointment. For urgent or emergent concerns after hours or on weekends, you are able to contact your doctor directly via personal phone. 


If you are not currently a member, you are welcome to stop by our office during business hours to discuss Membership inquiries, but you must be a member to be seen by our physicians.


We are open Monday through Thursday 8am - 4pm and Friday 8am-12pm.


We're unable to provide an exact cost for services before your visit with the physician. This is because the specifc care, tests, or procedures recommended will depend on the physician's evaluation of your individual needs at the time of the appointment. 


For certain surgeries or procedures, we can offer an estimate of our billed charges. However, we cannot guarantee the final out-of-pocket cost, as this will ultimately depend on how your insurance plan processes the claim.


We're happy to answer any questions and help you navigate your coverage as best as we can.


If a statement cycle has been mailed and it has been 30 days with no payment, the unpaid balance is considered past due.


 Same day appointments are often available especially for acute illness or injuries. Physicals and certain other appointments or procedures may book out a week or more. 


As required by insurance companies, we expect payment of “co-payments” before seeing the physician. Non-contracted insurance and deductibles are due and payable at the time of service.


Patients that are Self-Pay will be expected to pay their $50.00 Office Visit fee the same day of service. (Additional testing - patient responsibility 100% of cost, or 20% discount for those that pay in full on the same day of the visit.)


No. We cannot change the codes just so the insurance company will pay the claim. We have to follow national regulations on billing for physician services and our billing practices must be consistent with the documentation and services actually rendered by the physician. 


 Primary Care Associates, PC has an in-office clinical laboratory that provides testing for most lab tests your physician will order. There are some tests, however, that Primary Care Associates, PC cannot provide. For these tests, we send the specimen to RML for testing and RML will file your insurance. If your insurance does not pay the balance in full, then RML will bill you for the balance due. 


Our physicians are currently accepting new patients, however, our practice size is limited. If the physician you choose is at capacity, you are welcome to join their waiting list or consider another physician in our office. Visit MDVIP.com/doctor-search to confirm availability.

You can pay your medical bill at Primary Care Associates, PC at the check-in/check-out area or mail it to 510 SE Delaware Ave, Bartlesville, OK 74003. We accept Visa, MasterCard, or Discover payments by mail or phone. 


For MDVIP Membership payments, please use your MDVIP Connect Portal, or visit MDVIP.com and click "Express Pay" in the top right corner. For assistance with setting up your payments, please reach out to our office.


 Complete a release of information form. Once PCA receives the completed release of information form, your request will be processed and medical records will be forwarded as indicated. There is no charge to send medical records to other medical entities. There is a cost of $1.00 per page for all other requests (including the patient). 


Yes, we have a physician and front office staff that are fluent in Spanish. 


Your pharmacy can request your refill or you may call our office to request. Prescription refills may take 24-48 hours to be processed.
